Filling Out Camper Forms
You can find all forms in your CampInTouch account. They are due April 15th!

We encourage you to use the Campanion app to upload forms. It is quicker, easier, and you can use your phone! Just snap a photo of the form and upload it directly in the app–it will show up in your CampInTouch account.

Recent Camper Photo
REQUIRED
Please upload a current passport-style picture of your camper.
A picture should be uploaded in the “Forms” section of your Campanion app.
Bunk Requests
REQUIRED
If your camper would like to request to be in a bunk with other Campers in their grade, please use this form to make those requests.
Our system will only allow us to view two requests. We make every effort to honor bunk requests, and they are not guaranteed. We ask for your patience and understanding as we explore appropriate bunk configurations with a focus on building a warm and inclusive camp community.
The form can be completed using the Campanion app.
Camper Health History
REQUIRED
The form can be completed using the Campanion app or through your CampInTouch account.
Camper Immunization History
REQUIRED
Please upload your physician’s immunization forms or your state immunization form, signed by your physician with all required vaccines as outlined in the URJ Policy on Immunizations.
If your form includes the medical examination, please upload the form in both places.
This signed form must be scanned and uploaded to your Campanion app.
If this form is marked incomplete please check that the doctor has signed the form, the exam was administered after mid-August of last summer, and the date of the exam is on the form.
Camper Insurance Cards
REQUIRED
Please upload copies of the front and back of your camper’s insurance cards.
The cards must be scanned and uploaded to your Campanion app.
If this form is marked incomplete please re-upload both the front and back of your camper’s insurance card and make sure to preview the document in Campanion before you submit it. Many times only the front of the card is accidentally uploaded!
Camper Medical Examination Form
REQUIRED
Please upload your physician’s medical forms or state medical forms, signed by your doctor/physician.
If your form includes immunizations, please upload the form in both places.
This form can be completed using your Campanion app.
If this form is marked incomplete please check that the doctor has signed the form, the exam was administered after mid-August of last summer, and the date of the exam is on the form.
Camper Medical Authorization Form
REQUIRED
Please download from Campanion app or CampInTouch, print, sign, and upload this form.
The form must be scanned and uploaded to your Campanion app.
If this form is marked incomplete please upload a signed and dated Authorization form and make sure to preview the document in Campanion before you submit it. Many times a blank form is accidentally uploaded!
URJ Policy Statement on Immunizations
REQUIRED
This online form should be signed by a parent/guardian to acknowledge that you have read and understand the URJ vaccine policy.
This form can be completed using your Campanion app.
Camper Information Form
REQUIRED
This form (completed by a parent/guardian) allows you to share important information about your camper.
The form can be completed using the Campanion app or through your CampInTouch account.
Code of Conduct
REQUIRED
This online form should be signed by a parent/guardian AND camper to acknowledge that you have read and understand the policy.
This form can be completed using your Campanion app.
Action Plan
OPTIONAL
This form is required if your camper has severe allergies, has asthma, uses an inhaler, or has other medical needs. For campers with severe allergies, we prefer that you have your doctor fill out the form we have provided, but we will also accept action plans that your doctor provides. For campers with other medical needs, please use your doctor’s form.
Please download the form from the Campanion app or CampInTouch, and upload it after it is completed by a Physician.
If your camper does not have an Action Plan, there is no need to upload a document.
This form can be completed using your Campanion app.
Camper Physicals After Forms Due Date
OPTIONAL
Please fill out this form if your camper’s physical exam is scheduled after forms are due in mid-April so that we know when to expect your Camper Medical Exam Form and immunization history.
This form can be completed using your Campanion app.
B' Mitzvah Preparation Form
OPTIONAL
Please download from CampInTouch, print, and upload this form if your camper will become a B’ Mitzvah between August-December following camp and you would like them to review their material with our faculty while at camp. Your Rabbi, Cantor, Educator, or B’Mitzvah tutor will need to fill it out and sign it.
The form must be scanned and uploaded to your Campanion app.
Camper Travel Form
OPTIONAL
Submit this form if your camper will travel to/from camp either by plane or with an adult other than a parent/guardian.
This form can be completed using your Campanion app.
